A placeholder represents an instance of some entity such as patient or a specific observation.

Conceptual model of Placeholder metadata and instance data with relationship to other application data

Described from a UI perspective

A placeholder represents the Entities used in the visible application: Patient, result, problem, assessment, etc. It also indicates the GUI representation of that item both as a full page during a drilldown and when summarized such as on the patient tab (name, age, gender) for example.

A placeholder is characterized visually by having a close button. In addition the actual contents of the placeholder display is visualized by an pane called a drilldown.
